A groundbreaking diabetes management program developed in Collarenebri is empowering people to take control of their health and it could become a blueprint for communities across the country.

The initiative, the first of its kind in Australia, was created by Healthy Communities Foundation Australia (HCFA) Clinical Services Manager and proud Bundulung woman, Rishoniy Caine.

Currently participating in the Australian Primary Health Care Nurses Association’s Building Nurse Capacity Program, supported by the Australian Government, Ms Caine has designed a nurse-led, team-based model of care rooted in culture, connection, and practical support.

“This is the first holistic diabetes program of its kind in Australia,” Ms Caine said. “It’s grounded in culture, connection and community and that’s what makes it so powerful. We’re not just managing symptoms; we’re building knowledge and confidence for lifelong change.” […]
